A double-blind, placebo-controlled study published in the Journal of the International Society of Sports Nutrition (Talbott, S.M., et al., 2013, 10:28) supplemented 63 moderately stressed adults with 200 mg daily of a standardised root extract for four weeks and reported approximately 16 percent lower salivary cortisol together with improvements in tension, anger and confusion scores. A systematic review and meta-analysis published in Medicina (Leisegang, K., et al., 2022, 58(8):1047, doi:10.3390/medicina58081047) pooled five randomised trials and reported a significant increase in total testosterone (SMD 1.352, 95% CI 0.565 to 2.138, p = 0.001), with the clearest effects in men with hypogonadism or age-related decline and more modest, less consistent effects in younger men with normal baseline levels. Clinical protocols for this ingredient converge on 200 to 400 mg daily. The amount here sits at the lower end of that range rather than below it.
A note on standardisation. The trials above used specific standardised preparations. The Talbott study used an extract standardised to 22 percent eurypeptides and 40 percent glycosaponins; other trials used a freeze-dried aqueous extract, and clinical guidance commonly specifies 1 to 2 percent eurycomanone content. Tongkat ali extracts vary substantially in active compound concentration, and an extraction ratio alone does not indicate that concentration. L-arginine HCl, 50 mg. An amino acid the body uses as a substrate for nitric oxide synthesis.
